This form can be used for comprehensive claims when your entity’s covered auto or equipment repair estimates are $3,000 or less.

Simply complete all of the required fields below and attach an invoice/estimate with your submission, and you can immediately start repairs.

The NDIRF will process your claim on the first business day it is received. Repair payment checks are issued weekly to the member.
